Frequently asked questions

  • How many members can be covered in the ICICI Pru Group Suraksha Plus (GSP) plan?

    A minimum of 10 members are required at the time of policy inception. You can include any number of members between the ages of 18 and 85 years.

  • What is the frequency of interest accrual under the ICICI Pru Group Suraksha Plus (GSP) plan?

    The interest rate shall be declared in accordance with the Board approved policy. The interest rates shall be declared at regular intervals based on the fund size with appropriate disclosure on the website and shall be valid for a specified period as defined in the scheme rules. The interest shall be accrued at the end of the financial year on pro-rata basis.

Documents required for issue of policies

  • Complete filled Proposal Form with sign and stamp
  • Trust Deed and Trust Rules (Require LE Rules for LE [Leave Encashment] product)
  • CIT approval / CIT acknowledgement
  • Board Resolution + Trustee Resolution
  • Approved system quote copy + extension list [ if applicable ]
  • Trust PAN Card copy
  • Contribution Form
  • Final Member data
  • Signed copy of Factsheet
  • Onboarding docs (List provided below) - As per Trust and Company bifurcation

a. Company

  • Certificate of Incorporation/Certificate of commencement (issued by RoC or MCA)
  • Memorandum of Association and Articles of Association
  • PAN Card copy
  • Board Resolution to ascertain the Authorised individual (authorised signatory) or Power of Attorney
  • KYC of such Authorised Signatory (OVD + PAN/Form 60 + recent photograph)
  • Beneficial ownership declaration
  • KYC of beneficial owner(s) (OVD + PAN/Form 60 + recent photograph)

b. Trust

  • Registration certificate
  • Trust Deed
  • PAN/Form 60 of the Trust
  • Document naming the trustee (or other person acting on behalf of the trust) – this could be mentioned in the trust deed or through some other document)
  • KYC of such authorised signatory (OVD + PAN/Form 60 + recent photograph)
  • Beneficial ownership declaration
  • KYC of beneficial owner(s) (OVD + PAN/Form 60 + recent photograph)

Basic documents and verifications required for Claim Settlement

The claim payment will be as per Scheme Rules. The Master Policyholder will raise claims to avail Benefits with the following documents:

  • Claim intimation form
  • In case of death claim, following documents are required:
    1. Duly filled and signed Claimant Statement
    2. Recent photograph of the claimant
    3. Death Certificate issued by local government authority of the person insured in the policy (Life Assured)
    4. Signed copy of photo identity proof of the claimant
    5. Current Address proof of the claimant (Any one of the following: Aadhar Card, Valid Passport or Driver's License, Voters ID are considered as proofs)
    6. Signed copy of PAN card / Form 60 of the claimant
    7. Copy of cancelled cheque / bank statement / passbook of the bank account of the claimant where payment needs to be transferred.

    Additional documents will also be required, depending on the type of death, for faster processing of your claim –

    For deaths due to Natural/ Medical reasons: Death at hospital / Death at a place other than hospital

    1. Past medical records and treatment papers
    2. All hospitalization records such as Admission form, hospital treatment papers, Discharge summary, Diagnostic reports
    3. Medico-legal cause of death
    4. Employer Certificate – only if Life Assured was a salaried individual

    Death due to any other reason:

    1. First Information report (FIR)
    2. Postmortem report (PMR)
    3. Inquest/ Panchnama Report
    4. Viscera /Chemical Analysis Report, if any
    5. Final Police Investigation Report
    6. Newspaper Cutting, if any
    7. Driving License*
    8. Medical records and treatment papers
    9. All hospital Papers ( records such as Admission form, Indoor Case Papers (ICPs), Discharge summary, Diagnostic reports
    10. Duly filled in Medical Hospital Attendant Certificate
      Medico-legal cause of death
    11. Employer Certificate – only if Life Assured was a salaried individual
  • Any other documents or information as may be required by the Company for processing of the claim depending on the cause of the claim
  • The Company reserves the right to call for additional information, documents or particulars, in such form and manner as the Company would prescribe, and the Benefits would be paid only after receipt of such additional information, documents or particulars. * If death is due to road accident and the life assured was driving / riding the vehicle

All claims payments will be made in Indian currency in accordance with the prevailing exchange control regulations and other relevant laws and regulations in India

Inclusions

In the event of death of a Member, amount calculated as per the Scheme Rules will be paid in addition to the Sum Assured of Rs 10,000/-. Sum Assured under the product is guaranteed. The Sum Assured is payable on death of a member in addition to the applicable other benefit (i.e. the Gratuity benefit, Leave Encashment benefits, Post-Retirement Medical Benefits) as per the rules of the EB Scheme. In the event of death of the Member on Policy Renewal Date, then death benefit shall not be payable by the Company. The Member cover shall terminate with all rights and benefits thereunder. In the event of Employee/Member’s exit from Employer’s service, amount calculated as per the Scheme Rules for each Member is payable by the Company.. In the event of leave encashment while Employee/ Member is in service or at the time of leave encashment upon retirement of the Employee/ Member, amount calculated as per the Scheme Rules and applicable for each Member shall be payable by the company.
