What is “Personal Health Declaration”?

Personal Health Declaration is a form that you submit to keep us updated on the condition of your health^*. It enables us to be aware of any changes in your health from the time when you last submitted a health declaration.

This form is a mandatory requirement for policy revival. For best insurance experience, you may be asked to undergo health check-ups. Further information on this will be communicated to you as and when required.
